PROBLEM TO BE SOLVED: To provide a similar image retrieval device that is reliable and can retrieve similar images reasonably consistent with similarity perceived by human vision, operation method of the similar image retrieval device and similar image retrieval program.SOLUTION: A feature calculation unit 61 analyzes an inspection image 19 and calculates a feature Zx corresponding to a lesion pattern. A probability calculation unit 62 uses a calculation formula 66 to calculate a first existence probability Px, which is a probability that the lesion pattern exists in the inspection image 19. The calculation formula 66 is created in advance by a statistical method on the basis of a relationship of the feature with presence or absence of the lesion pattern in a learning image visually determined by humans.
