ADDRESS LABEL HAVING ADDRESS CERTIFICATE FORM PIECE

PROBLEM TO BE SOLVED: To improve the efficiency of sticking work and to prevent the occurrence of sticking errors and delivery errors by successively providing the rear side of a slip form with an adhesive layer and release paper for covering the adhesive layer and peelably providing the adhesive layer with the address certificate form piece. SOLUTION: The address label is formed by separably segmenting and connecting the slip form 2 via a slit 3 to the address certificate form piece 2a having at a receiver address entry part and the delivery certificate form piece 2b having a receipt seal column. The adhesive layer 8 is formed on the rear surface side of the slip form 2, i.e., the rear surface side to the address certificate form piece 2a and the delivery certificate form piece 2b and the surface of the adhesive layer 8 is releasably coated with the release paper 9. A release layer 10 is formed between the rear surface side of the delivery certificate form piece 2b and the adhesive layer 8 in order to make the delivery certificate form piece 2b peelable from the adhesive layer 8. The label is so constituted that only the delivery certificate form piece 2b may be peeled from the adhesive layer 8 after the slip form 2 is stuck to a delivery object by the adhesive layer 8.
