PROBLEM TO BE SOLVED: To facilitate the regulation of an adhesive strength when a form base material is separated from an adhesive layer by forming a separation layer on the lower surface of a form section of the form base material, and forming an adhesive strength suppressing layer which suppresses the adhesive strength of the adhesive layer on at least one section between the separation layer and the adhesive layer. SOLUTION: An adhesive strength suppressing section 13c is formed between a separation layer 16 and a first adhesive layer 12 on the rear surface of a delivery form section 11a, and includes a separation trigger section, and is provided in a manner to be solidly printed in the carrying direction at the time of printing. The section 13c is provided in a manner to be solidly printed, and the delivery form section 11a is not adhered, and the delivery form section 11a can be easily peeled. Also, the section 13c has a shape which meets the carrying direction at the time of printing, and even if the delivery form section 11a is lifted from the section 13c, the carrying is not affected, and the delivery form section 11a is prevented from being peeled by being caught by a printer.
