The present invention relates to a planar wave-guide element and comprises; a first cylindrical lens which is arranged with a z-shaft as the center and collimates beams outputted from multiple emitters of a laser diode bar; a lens array which collects the beams outputted from the each emitter passing through the first cylindrical lens; multiple first wave-guides which exist on an x-y plane as the number of the emitters and are collected to one portion after passing through a bending section; a taper which connects the each first wave-guide to the lens by being narrowed towards a first wave-guide direction; a wave-guide which combines the multiple first wave-guides into one.
