<p><P>PROBLEM TO BE SOLVED: To provide a patient notebook which can check past dental treatment records by collectively keeping treatment plans, cure guidance and recovery progress on the parts of a dentist and a patient. <P>SOLUTION: This easy-to-carry patient notebook has past treatment details and cure guidance data to be kept for reference henceforth on the part of the patient as well as the dentist. In addition, the desk pad contains described entry samples and color coding in order to eliminate an entry error in a printed form and thus clarifies the points to be basically entered by the dentist, hence no missing entry. Consequently, the notebook and the desk pad can guarantee the obligations of the dentist, in the light of obligatory issuance of a document furnishing information on the part of the dentist, effective as from April, 2006. Further, so many pieces of the copying paper 4 are provided in the form of a bundle according to an estimate of the average number of the paper 4 to be consumed most frequently per year. On the other hand, the filled form to be kept for the past dental treatment records on the part of the patient, is clearly legible by color coding, provided, however, the above practice is one case example for dentistry and there is no binding on the way of the practical application. <P>COPYRIGHT: (C)2008,JPO&INPIT</p> |
