METHODS FOR THE SURGICAL APPLICATION OF A FASTENER AND THE ENDOLUMINAL TREATMENT OF GASTROESOPHAGEAL REFLUX DISEASE (GERD)

A surgical instrument (200)includes an end effector (202) having a clevis (224) and first and second jaws (226, 228) mutually rotatable between open and closed positions. The jaws (226, 229) are proximally directed and laterally displaced relative to a longitudinal axis of a control shaft (206) of the instrument. The jaws (226, 228) hold first and second parts (12, 14) of a fastener (10), respectively. The first part (12) includes a base (18) having upstanding tissue piercing posts (32, 34), and the second part (14) includes another base (54) defining apertures (58, 60) for receiving the posts (32, 34), as well as a portion (52) movable relative to the second base (54). When the upstanding posts (32, 34) are inserted into the apertures (58, 60), the movable portion (52) can be moved into a second configuration to lock the parts (12, 14) of the fastener (10) together. The instrument (200) is adapted to move the second part (14) into the second configuration.
