The instrument for holding and guiding of needles or sutures is designed for guiding them through a trocar such that the suture can be pulled out of the instrument, intra-corporeally knotted and tightened. The instrument consists of handling elements in two regions (A, B). In the third functional region (D) is a guide tube (10) and protruding, or slidable fixing elements (1,51). In this region is also provided an extensible knotting slider (6) a fixing element (5) for the suture (13) is so mounted at the other element (10), or the guide tube, that it can be guided about the other element tip (2).
