PROBLEM TO BE SOLVED: To provide a heat insulating plate doubling as a form, which serves as the form when concrete is poured, and a heat insulating material and a substrate, and allows concrete to make positive contact with a contact surface thereof, and to provide a method of mounting the same. SOLUTION: A plate body A doubling as the form consists of a heat insulating plate member 1 and a joint member 2. The plate member 1 is made of a foamed plastic and shaped like a flat plate. The joint member 2 is a longitudinal member having an almost H-shaped cross section and a pair of insertion slits S, S formed in both side surfaces of a main body 20 in a manner opening outward, wherein edges 21a, 21a of both side walls 21, 21 defining each slit S are cut like a bell mouth. Then the concrete poured into an area enclosed by the plate bodies A, A, etc. flows into dovetail grooves Q and hardened in the same, and therefore the plate body A and the concrete are rigidly secured to each other in a manner engaging with each other.
