PROBLEM TO BE SOLVED: To provide a four-cycle reciprocating piston engine equipped with a supercharging piston capable of keeping dynamic balance of the engine and outputting high power with simple structure. SOLUTION: This four-cycle reciprocating piston engine equipped with a supercharging piston comprises total two pairs of two-way reciprocating output piston that are attached in the same phase to both ends of a crank shaft having three inline crank pins, and a pair of two-way reciprocating supercharging piston that is attached to the crank pin in the middle in the phase rotated 180 degrees with respect to the two pairs of two-way reciprocating output piston. This engine also comprises a cylinder head having an inlet opening to a two-way reciprocating supercharging piston cylinder, a compressed air supply opening from the two-way reciprocating supercharging piston cylinder, a compressed air intake opening to each two-way reciprocating output piston cylinder, and an outlet opening from the each two-way reciprocating output piston cylinder.
