PROBLEM TO BE SOLVED: To supply each driving current to a focus coil and a tracking coil mounted on a lens holder via four suspension wires. SOLUTION: Four terminals 14d in total are projected upper and lower parts of both left and right sides of the lens holder 14, any of each end section of the four suspension wires 15 is electrically connected with an end part of the focus coil 18 mounted on the lens holder 14 or an end part of the tracking coil 19 at regions of the four terminals 14d, a cable support 11e4 for supporting four cables 23 for power supply is formed on a suspension section section 11e having insulation when each of other ends of the four suspension wire 15 is freely oscillatably supported on the suspension section 11e and each of other ends of the four suspension wires 15 is electrically connected with the four cables 23 for power supply at regions of suspension.
