PROBLEM TO BE SOLVED: To provide a channel forming form enabling continuous construction using mechanical placing for forming a channel even in case of a side wall surface of a preceding finished existing structure, including repairing case, and a channel construction device and channel construction method using the channel forming form. SOLUTION: The channel forming form fitted to a movable channel forming vehicle is formed by a long outer form having a groove like section and formed by side walls and a top plate connecting the side walls, and a cylindrical long inner form disposed inside the long outer frame and connected to the back of the top plate. A hopper fitting part formed by cutting a part of the top plate of the front side of the central part in the longitudinal direction of the long outer form and the long inner form positioned below the hopper fitting part are cut off with the end part closed, the cut-off part is taken as a space part, and the space part is taken as a vibration exciter insert part communicating with the gap part in the outer peripheral part of the long inner form.
