PROBLEM TO BE SOLVED: To obtain a pharmaceutical, containing a compound having a specific structure as an active ingredient and capable of lowering serum cholesterol. SOLUTION: This pharmaceutical comprises a compound represented by the formula [R<1> is H or a bivalent moiety which distances L from the ring B by 6-8 intervening atoms; R<2> is H, a 1-5C straight chain or branched chain (un)saturated chain, etc.; R<3> is absent or a (un)substituted bivalent moiety, etc., which join R<2> and L to form a 5- to a 7-membered heterocyclic ring; R<4> is H or a lower alkyl; R<5> and R<6> are each H, hydroxy, etc.; L is -CON< or -N<; the dotted line in the B ring is an optional bond] or its salt and, as necessary, an estrogen. A compound in which L is -N< and R<2> joins R<3> and L to form piperidino is preferred as the compound represented by the formula. The compound represented by the formula is preferably administered in 50-600 mg daily dose to a human in one to three divided portions. The daily dose of the estrogen is preferably 0.1-2 mg administered in one to three divided portions.
