PROBLEM TO BE SOLVED: To provide a manufacturing form for a PC floor slab and the like which enables the common use of a supporting plate by blocking a window installed to a peripheral frame with a window burying plate which is formed by stacking, on the supporting plate having long holes, a middle fishplate which fits in between the space of a reinforcing bar and an upper fishplate and a lower fishplate both of which have through-holes for the reinforcing bar opened therein and accordingly if the fishplates are changed according to the change of the space of the reinforcing bar. SOLUTION: The form forms a PC floor slab and the like which holds the reinforcing bar 4 in the form A so as to hang over outward from the peripheral frames 2, 3 and, when concrete is placed, makes the linking reinforcing bar project from the periphery of a main body. In the peripheral frames of this form, the overhang part of the reinforcing bar being regarded as the window 5, the window 5 is blocked by the window burying plate 10 which is formed by stacking, on the supporting plate 6 which has a plurality of long holes 12 formed therein and matches the maximum space of the reinforcing bars arranged at equal spaces, a middle fishplate 7 formed into the width accommodated to various spaces of the reinforcing bars arranged at the equal spaces and an upper fishplate 8 and a lower fishplate 9 which are vertically disposed on the middle fishplate and have the through-holes 14 in the inner edge side for passing the reinforcing bars.
