PROBLEM TO BE SOLVED: To provide an optical pickup module which prevents generation of noise due to rotation of gears and is constituted of a small number of components, and to provide an optical disk mounted with the optical pickup module. SOLUTION: A carriage 7 is held by a guide shaft 11 freely movably. Rotation of a feed motor 12 is transmitted to a screw shaft 14 and the screw shaft rotates. A rack 15 moves along the screw shaft 14, and thereby the carriage 7 moves between the inner circumference side and the outer circumference side of the optical disk. The feed motor 12 is directly coupled to the screw shaft 14 to rotate it without using gears. The screw shaft 14 therefore is rotated without rotary torque of the feed motor 12 converted.
