PROBLEM TO BE SOLVED: To provide a delivery slip which uses a non-impact printer, when printed information is entered by a printer and is handled in such a way that when a delivery goods is brought back because of the absence of an address, the released delivery slip combined with a receipt slip can be kept in custody in relation to the delivery goods. SOLUTION: The delivery slip 1 has a self-adhesive applied to the back and also that self-adhesive applied to the face side of an attachable base material covered with a releasable paper 4, in such a way that the respective delivery information entry sides of the attachable slip and the delivery slip combined with the receipt slip are exposed, and also that the delivery slip combined with the receipt slip is releasable from the attachable base material. In addition, a non-releasable part 4a for securing an insertion space in which the delivery slip combined with the receipt slip released from the attachable base material, is inserted and retained when the releasable paper 4 is released and the attachable base material is bonded to the delivery goods, is demarcated and formed on the releasable paper 4 with a zipper line 40.
