An oral sustained release pharmaceutical dosage form including: a tablet including an analgesically effective amount of hydromorphone or a pharmaceutically acceptable salt thereof; a coating surrounding said tablet, the coating including a hydrophobic polymer and the coating having a passageway disposed therein prior to oral administration; and the dosage form providing an analgesic effect for at least approximately 24 hours after oral administration to a human patient. The dosage form upon single dose oral administration to a human patient providing a time to maximum plasma concentration (Tmax) of hydromorphone in 2 to 15 hours, or providing an absorption half-life of hydromorphone of 1 to 8 hours.
